I'd check if the body text field is empty. GitHub's issue form requires a non-empty description before enabling the submit button. Type something in the body field, even a brief summary. Also confirm both dropdowns have a selected value - sometimes the placeholder text isn't a valid selection. If those are filled and it still won't submit, open your browser's developer console (F12) to see if there's a JavaScript error blocking the form.
